Qualifications

• Master’s degree in Social Work (MSW) or Master of Social Services (MSS) from an accredited program.
• Licensed CSW or MSW as required by Delaware state law.
• Acute care social work experience preferred.

Responsibilities

• Participate in the Care Management Triad Team (RN Case Manager, Social Worker, Utilization Management) to coordinate patient discharge and progression.
• Address psycho-social barriers and support discharge needs.
• Review patient status for appropriateness and payer coverage.
