ChristianaCare is seeking an energetic Social Worker to join our Care Management Triad Team. You will work with an RN Case Manager to drive patient progression, establish discharge plans, and resolve psycho-social barriers to discharge.

Qualifications

• MSW or MSS required
• Licensed CSW or MSW as required by Delaware
• Acute Care Social Work experience preferred

Responsibilities

• RN Case Manager – manage patient care and drive discharge planning.

• Utilization Management – review patient status for appropriateness and payer coverage.
