Job summary

ChristianaCare is seeking an energetic, compassionate Social Worker to join a care management team. A $15,000 sign-on bonus is available for a two-year commitment, with the opportunity to impact patient flow and discharge planning across the organization.

The Care Management Triad collaborates RN Case Manager, Social Worker, and Utilization Management to optimize patient outcomes. The role requires a MSW/MSSW and Delaware licensure, with acute care experience preferred.

Qualifications

• Master’s degree in Social Work (MSW) or MSS from an accredited graduate school required prior to hire date.
• Licensed CSW or MSW as required by the state of Delaware prior to hire date.
• Acute Care Social Work experience preferred.

Responsibilities

• RN Case Manager – manage patient care and discharge plan.
• Social Worker – resolve psycho-social barriers and supports discharge needs.
• Utilization Management – review patient status for appropriateness and anticipated payer coverage.
