Job summary

Ascension is seeking an experienced Registered Nurse with a Case Management background to coordinate patient transitions at Ascension St. John’s Medical Center in Tulsa, OK.

You will collaborate with clinical teams to improve patient outcomes, ensure timely care, and optimize resources across the care continuum.

Qualifications

• RN licensure obtained prior to hire date or transfer date.
• CCMC certification preferred; ACM accepted.
• Diploma from an accredited nursing program; professional licensure required at hire.

What minimum requirements you’ll need

Licensure / Certification / Registration:

• Registered Nurse obtained prior to hire date or job transfer date required.
• Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

• 3+ years of RN experience
• Case Management experience preferred
