Job summary

Ascension St. John Medical Center in Tulsa, OK is hiring a Case Manager in Total Quality Management to coordinate discharges to home or rehab and arrange follow-up care.

You will connect families with community health programs and financial aid, coordinate equipment needs, and work with insurance to speed approvals while reducing unnecessary hospital stays. This full-time role supports safe, cost-effective patient care.

Qualifications

• Licensure / Certification / Registration: BLS Provider preferred; CCMC preferred; Counselor preferred; Social Worker preferred.
• Education: High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Responsibilities

• Patient Care and Safety: Plan patient discharges from hospital to home or rehab and coordinate follow-up care.
• Resource Connection: Connect families with community health programs and financial aid and arrange necessary medical equipment for home use.
• Cost and Efficiency: Help reduce hospital costs by preventing unnecessary stays and speed up care approvals with insurers.
• Team Communication: Coordinate with doctors, nurses and social workers and keep patients and families updated on treatment plans.

Education

High School diploma equivalency with 2 years of cumulative experience; Associate's degree/Technical degree; or 4 years of applicable cumulative job specific experience

How you'll make an impact in this role

• Cost and Efficiency: The Case Manager cuts hospital costs by preventing long, unneeded stays and works with insurance companies to speed up approval for care.
• Team Communication: The Case Manager connects doctors, nurses and social workers for better teamwork and keeps patients and families updated on the treatment plan.

What Minimum Requirements You'll Need

Licensure / Certification / Registration:

• BLS Provider preferred. American Heart Association or American Red Cross accepted.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
• Counselor preferred.
• Social Worker preferred.

Education

• High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
