Job Board in New York, NY is seeking a Team Leader (Social Work Supervisor) for the Community Guardianship Program. The role manages a multidisciplinary team to meet client needs, ensure court orders and contractual obligations are met, and maintain high-quality service delivery.

The supervisor coordinates care with health professionals, supports staff training, and oversees hiring while ensuring monthly client contact and adherence to policies.

Formación

• MSW or equivalent; or four to ten years related experience and/or training; or equivalent combination of education and experience
• Supervisory experience preferred
• Computer literate; ability to work with a range of programs including financial databases and MS Office
• Excellent verbal and written skills; able to multi-task and work independently with attention to detail

Responsabilidades

• Lead and supervise team including caseworkers, accountants on client fiscal issues, and coordinators on benefits and coordination
• Consult with Program Managers and keep them informed of activities, needs, and problems related to case management and court orders
• Supervise social work interns as assigned and conduct annual performance reviews
• Initiate client case conferences and assist with development/implementation of service plans
• Collaborate with physicians, nurses, and health care staff in client evaluation and treatment
• Liaise withAdult Protective Services and coordinate emergency coverage and on-call supervision (24/7)
• Assist in hiring/discharging staff and monitor Medicaid certifications and recertifications
• Ensure clients are seen at least once per month and uphold agency policies and training requirements
