AltaMed Health Services Corp. seeks a PACE Social Work Supervisor to oversee the social work team at the assigned site, coordinating assessments and care plans, and promoting interdisciplinary collaboration for person-centered care.

The role requires an MSW, 2+ years of supervisory experience, and bilingual skills. A valid BLS certification is preferred. AltaMed offers competitive compensation, benefits, and growth opportunities.

Qualificações

• MSW degree from an accredited university.
• Minimum of 2 years' supervisory experience in health/human services.
• Minimum of 2 years in case management/social advocacy/care planning with geriatric population preferred.
• Bilingual Spanish/English, Chinese/English, or another second language strongly preferred.
• A valid BLS certification or higher per AHA/Red Cross guidelines.

Responsabilidades

• Oversee routine assessments and care planning; supervise direct reports.
• Coordinate family conferences and community referrals; monitor documentation.
• Onboard and orient new hires; conduct competency and performance evaluations.
• Provide guidance on participants' behavioral health needs and life planning.
