Job summary

High Desert PACE seeks a Social Work Manager to lead daily social work services for participants and caregivers. You will coach staff, monitor caseloads, and coordinate with the interdisciplinary team to address psychosocial needs impacting health, safety, independence, and quality of life.

You will oversee assessments, care planning, and service coordination, ensuring compliance with CMS and state requirements.

Qualifications

• MSW from an accredited school of social work.
• At least three years of social work experience in health care or aging services, incl. one year with frail or elderly adults.
• Current California license or registration where required.

Responsibilities

• Lead daily social work service delivery for High Desert PACE participants and caregivers.
• Supervise staff, monitor caseloads, and ensure timely documentation.
• Collaborate with IDT to address psychosocial needs affecting health and quality of life.
• Conduct psychosocial assessments and care planning within required timeframes.
• Coordinate with nursing, primary care, and community partners during transitions.
