Kaiser Permanente is seeking a qualified social worker for inpatient and care coordination roles across the Mid-Atlantic region. You will collaborate with health care teams to coordinate cost-effective services aligned with each member's plan of care, aiming to enhance independence and quality of life.

The role requires an MSW and at least 1 year of clinical social work experience in health care, plus multi-state credentials with licensure across DC, MD, and VA. A $7,500 sign-on bonus is offered.

Qualificações

• MSW required.
• 1 year of clinical social work experience in a health care setting.
• Credentials from multiple states required; primary state credentials at hire; additional post-hire credentials may be required.

Responsabilidades

• Collaborate with health care team to coordinate services per member's plan of care.
• Perform psychosocial assessments to inform goals, support, and resources.
• Develop and monitor a plan of care with the nurse case manager to promote optimal functioning.
• Identify and facilitate community resources to meet care plan cost-effectively.
• Document case management activities consistently per organization standards.
• Analyze outcomes to identify improvements in program quality and cost effectiveness.
• Assist members with access to local, state, and federally funded programs (e.g., Medicaid, Medicare).
• Provide supportive counseling and education to members, families, and caregivers.
• Promote knowledge of case management standards and NASW ethics; schedule family meetings as needed.
• Complete guardianship paperwork and provide technology support for virtual court attendance.
• INPATIENT ONLY - UAIS form for long-term care (Virginia Medicaid requirement) where applicable.
• Performs other related duties as assigned.
