Job summary

Kaiser Permanente in Gaithersburg, MD, is seeking a Social Worker to collaborate with the health care team to coordinate cost-effective services aligned with the member's plan of care, promote independence, and enhance quality of life.

Responsibilities include psychosocial assessments, referrals to community resources, Medicaid/Medicare guidance, and supporting patients and families through complex decisions. Requires an MSW and multi-state licensure within 6 months.

Qualifications

• Minimum 1 year of clinical social work experience in a health care setting.
• MSW degree required.
• Credentials from multiple states with licensure within 6 months of hire (DC/MD/VA as applicable).

Responsibilities

• Problem identification, psychosocial assessment, financial counseling/referral, accessing community resources, placement for care, and guidance through health-related legal processes.
• Develop and monitor a plan of care with the nurse case manager to promote optimal functioning and quality of life.
• Identify and advocate for organizational and community resources to meet the plan of care in a cost-effective, timely manner.
• Document case management activities consistently in accordance with organizational standards.
• Assist with applying for local/state/federal programs (Medicaid, Medicare, Disability) or refer to community agencies as needed.
• Provide supportive counseling and education to members, families, caregivers, and health care team, including end-of-life issues and Advanced Directives.
• Schedule and coordinate family meetings; complete guardianship paperwork and provide technology support for virtual court attendance.
• Inpatient: complete Uniform Assessment Instruments (UAIS) for Virginia Medicaid when applicable.
