Conduct comprehensive psychosocial assessments, evaluate patient and family needs, and develop/coordinate care plans that connect members to appropriate medical, behavioral health, and community resources.

Work closely with physicians, nurses, and interdisciplinary teams to support safe transitions of care, strengthen continuity, and help reduce readmissions while improving quality outcomes.
