Kaiser Permanente in Washington state seeks a social worker to provide a wide range of clinical services in primary care, inpatient, or home health settings. You will evaluate psychosocial assets and deficits and coordinate a plan of care with the care team to help patients maintain independence.

Responsibilities include brief counseling, advocacy, referrals, and care coordination within a multidisciplinary team.

Competenze

• Minimum one (1) year of previous social work experience.
• Masters degree in Behavioral Science or Masters in Social Work.
• Licensed in Washington: LMFT, LMHC, LCSW, or LCSW-C (Washington)
• Excellent written and verbal communication skills.

Mansioni

• Provide social work interventions including psychosocial assessment, brief individual or family, advocacy, referrals, and care planning.
• Coordinate with multidisciplinary team and participate in family or care conferences.
• Assess patient and family needs, communicate with providers, and adjust care plans accordingly.
