Job summary

Qualifications

• Minimum one year of clinical social work experience in a health care setting.
• Masters degree in social work (MSW) required.
• Credentials from multiple states with the primary state at hire and additional post-hire as needed.

Responsibilities

• Collaborate with the health care team to coordinate appropriate, cost-effective services aligned with members’ care plans.
• Complete comprehensive psychosocial assessments to evaluate goals, supports, resources, status, and response to treatment.
• Develop and monitor a plan of care with the nurse case manager to promote optimal functioning and quality of life.
• Identify and advocate for organizational and community resources to meet the care plan in a cost-effective manner.
• Document case management activities consistently per organizational standards.
• Analyze outcomes to identify improvements in program quality and cost effectiveness.
• Assist with applying for local, state, and federal programs (e.g., Medicaid, Medicare) and refer to community agencies as needed.
• Provide supportive counseling and education to members, families, and caregivers, including end-of-life issues.
• Maintain knowledge of laws/regulations in the local market and NASW Code of Ethics.
• Schedule and coordinate family meetings; complete guardianship paperwork as needed.
• Perform other related duties as assigned.
