Maryland Care Management, Inc. (MCMI) seeks an enthusiastic Community Health Worker to coordinate care for Medicaid members across home, clinic, and community settings. You will educate, support, and link members to essential services and resources, while coordinating with providers and social services teams.

You will work in a hybrid setup with 3 onsite days per week and travel up to 75% to Cumberland, MD. Prior CHW experience and knowledge of care coordination are highly valued.

Qualificações

• Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services.
• Ability to plan, implement, and evaluate individual member care plans.
• Knowledge of transportation and other barriers to care that may be encountered by the member.
• Ability to communicate medical information to health care professionals and care coordinators over the telephone.
• Experience with Medicaid populations is common in this role.

Responsabilidades

• Assist members in their homes, community, or clinic setting and communicate program purposes and impact.
• Document all member encounters and contracts; submit monthly reports; maintain electronic files.
• Educate members on proper use of Emergency Department and alternatives; coach on chronic condition management.
• Assist members in assessing health-related services, including obtaining a medical home and overcoming barriers to care.
• Provide advocacy during initial medical visits to ensure needs and referrals are conveyed.
