Qualifications

• Two years of job-related experience in long-term care is preferred.
• Bachelor’s degree in social work or related field required.
• Experience with social work, preferably with patients and families in a nursing facility.
• Detail oriented, computer literate and strong communication skills.

Responsibilities

• Manage and direct the Social Services Department and address residents social, emotional, and psychological needs.
• Assess, document and provide interventions for psychosocial needs of all residents.
• Develop care plans based on assessments.
• Maintain relationships with the medical profession and other health facilities.
