Qualificações

• Bachelor’s degree in Social Work or related field; LSW preferred.
• Experience in skilled nursing or healthcare setting.
• Strong knowledge of state and federal regulations (MDS, discharge planning, etc.).

Responsabilidades

• Oversee and coordinate social services for both short-term rehab and long-term care residents.
• Conduct psychosocial assessments and care planning.
• Serve as a liaison between residents, families, and interdisciplinary teams.
• Ensure compliance with federal, state, and facility policies.
