Job summary

Pearl Healthcare is seeking a Social Services Director in the Schaumburg, IL area to own resident well-being and drive psychosocial care from admission through discharge.

You will lead care conferences, coordinate individualized service plans, and guide residents and families through transitions while ensuring regulatory compliance and high-quality standards.

Qualifications

• Bachelor’s degree in Social Work or Human Services required or preferred.
• At least 2 years of long-term care or healthcare social work experience.
• Strong understanding of discharge planning and case management.
• Empathy, professionalism, and a solutions-focused mindset are essential.
• Leadership experience strongly preferred.
