Job summary

Skilled Healthcare Center is seeking a Social Services Director to own resident well-being in our post-acute setting in the Chicago area. You will lead the psychosocial care process from admission to discharge and coordinate with families and the care team.

You’ll oversee discharge planning, case management, and ensure regulatory standards are met, while guiding a compassionate social services staff in a mission-driven, team-oriented culture.

Qualifications

• Bachelor's degree in Social Work or Human Services with preferred licensure (LSW/LMSW)
• 2+ years in long-term care or healthcare social work
• Strong discharge planning and case management knowledge
• Empathy, professionalism, and solutions-focused mindset
• Prior leadership experience preferred
