Qualifications

• Licensed Social Worker in the state.
• Bachelor’s degree in Social Work; MSW preferred.
• At least four years in a medically related long-term care setting.
• At least two years in a supervisory role in social work.
• Knowledge of community resources.
• Excellent English communication, written and verbal.
• Flexible, adaptable, organized and detail-oriented.
• Ability to handle confidential information with discretion.

Responsibilities

• Serve as facility representative with prospective residents, families and discharge planners.
• Provide social assessments on admission and identify psychosocial needs per regulations.
• Assist with orientation of residents to the facility and services.
• Record admission data including social, medical and financial information and address concerns.
• Maintain documentation in residents' health records for the social services care plan.
• Ensure care plan documentation meets regulatory needs.
• Participate in care planning meetings with interdisciplinary team.
• Assist residents/families in selecting new physicians as needed.
• Communicate admission/discharge information to departments.
• Advocate for residents/families and safeguard resident rights.
• Maintain regular family contacts and liaison with care team.
• Establish regular family support meetings for medically related needs.
• Explain government and facility policies to new residents/families.
