Job summary

Avantara Norton is seeking a Social Services Director to lead resident social and emotional care in a Skilled Nursing setting. You will ensure residents are prepared for their future, coordinate discharges, and participate in care conferences to uphold high standards.

The ideal candidate holds a bachelor’s degree in Social Work or a related field, with at least two years in a Social Worker role (Healthcare preferred); graduate-level qualifications and prior director experience are preferred but

Qualifications

• Bachelor’s degree in Social Work or similar field
• Two years of experience in a Social Worker role, preferably in a healthcare setting
• Social Services Director experience in a Skilled Nursing Facility preferred
• Graduate degree in Social work or similar field preferable

Responsibilities

• Prepare residents for their future and ensure high standard of care
• Coordinate resident discharges and participate in resident care conferences
• Address residents' social and emotional needs during their stay

As a Social Services Director you will prepare our residents for their future. Your work will ensure our residents receive the high standard of care they have grown to expect at Avantara Norton by ensuring our residents social and emotional needs are met during their stay, coordinating resident discharges and participating in resident care conferences. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

As a minimum, a bachelor’s degree in Social Work or similar field and two (2) years of experience in a Social Worker role preferably in a healthcare setting. Prior Social Services Director experience in a Skilled Nursing Facility, Graduate degree in Social work or similar field and five (5) years of related experience is preferable but we are willing to invest in the right candidate!
