Job summary

Longterm Health Management Services in Chicago, IL is seeking a Social Services Director to lead resident social well-being and coordinate discharge planning within our skilled nursing environment.

The ideal candidate holds a Bachelor's in Social Work (or related field) with two years of social work experience, preferably in healthcare; a graduate degree and five years of related experience are preferred but we will invest in the right candidate.

Qualifications

• As a minimum, a Bachelor's degree in Social Work or similar field and two (2) years of experience in a Social Worker role preferably in a healthcare setting.
• Prior Social Services Director experience in a Skilled Nursing Facility, Graduate degree in Social work or similar field and five (5) years of related experience is preferable but we are willing to invest in the right candidate!

Responsibilities

• Prepare residents for their future and ensure social and emotional needs are met during their stay.
• Coordinate resident discharges and participate in resident care conferences.
• Ensure residents are comfortable and safe through knowledge and attention to detail.

As a Social Services Director you will prepare our residents for their future. Your work will ensure our residents receive the high standard of care they have grown to expectby ensuring our residents social and emotional needs are met during their stay, coordinating resident discharges and participating in resident care conferences. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.
