Job summary

Avantara Norton is seeking a Social Services Director to join our skilled nursing team dedicated to high-quality resident care.

You will lead social services, coordinate resident discharges, and participate in care conferences, ensuring residents' social and emotional needs are met.

Requirements include a bachelor’s degree in Social Work, at least 2 years as a Social Worker in healthcare, with Director experience preferred; graduate degree is a plus.

Qualifications

• Bachelor’s degree in Social Work or a related field required.
• Two (2) years of experience in a Social Worker role, preferably in a healthcare setting.
• Prior Social Services Director experience in a Skilled Nursing Facility is preferable; graduate degree is a plus.

Responsibilities

• Lead and manage social services for residents.
• Coordinate resident discharges and participate in resident care conferences.
• Ensure residents' social and emotional needs are met during their stay.

As a Social Services Director you will prepare our residents for their future. Your work will ensure our residents receive the high standard of care they have grown to expect at Avantara Norton by ensuring our residents social and emotional needs are met during their stay, coordinating resident discharges and participating in resident care conferences. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

As a minimum, a bachelor’s degree in Social Work or similar field and two (2) years of experience in a Social Worker role preferably in a healthcare setting. Prior Social Services Director experience in a Skilled Nursing Facility, Graduate degree in Social work or similar field and five (5) years of related experience is preferable but we are willing to invest in the right candidate!
