Job summary

Legacy Healthcare is seeking a Social Services Director to lead social work services in a skilled nursing environment. You will oversee resident social needs, coordinate discharge planning, and participate in care conferences to ensure high-quality resident well-being.

The ideal candidate holds a Bachelor's degree in Social Work or a related field with at least two years of social work experience in healthcare, and prior Social Services Director experience is preferred but not required.

Qualifications

• Bachelor's degree in Social Work or similar field
• Two years of experience in a Social Worker role, preferably in a healthcare setting
• Prior Social Services Director experience is preferable but the company is willing to invest in the right candidate

Responsibilities

• Prepare residents for their future; ensure social and emotional needs are met during their stay
• Coordinate resident discharges
• Participate in resident care conferences

As a Social Services Director you will prepare our residents for their future. Your work will ensure our residents receive the high standard of care they have grown to expectby ensuring our residents social and emotional needs are met during their stay, coordinating resident discharges and participating in resident care conferences. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

As a minimum, a Bachelors's degree in Social Work or similar field and two (2) years of experience in a Social Worker role preferably in a healthcare setting. Prior Social Services Director experience in a Skilled Nursing Facility, Graduate degree in Social work or similar field and five (5) years of related experience is preferable but we are willing to invest in the right candidate!
