Job summary

Longterm Health Management Services seeks a Social Services Director to join a friendly, caring team in a skilled nursing setting. • Graduate degree in Social Work or similar field and five years of related experience are preferable

Responsibilities

• Prepare residents for their future and ensure their social and emotional needs are met during their stay
• Coordinate resident discharges
• Participate in resident care conferences
• Apply knowledge, skills and attention to detail to ensure residents are comfortable and safe

As a Social Services Director you will prepare our residents for their future. Your work will ensure our residents receive the high standard of care they have grown to expectby ensuring our residents social and emotional needs are met during their stay, coordinating resident discharges and participating in resident care conferences. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

As a minimum, a Bachelors's degree in Social Work or similar field and two (2) years of experience in a Social Worker role preferably in a healthcare setting. Prior Social Services Director experience in a Skilled Nursing Facility, Graduate degree in Social work or similar field and five (5) years of related experience is preferable but we are willing to invest in the right candidate!
