Job summary

Legacy Healthcare is seeking a Social Services Director to join our skilled nursing team. You will lead social work services, support residents and families, and help coordinate care across departments to uphold the quality of life and safety our residents expect.

Ideal candidates hold a Bachelor’s degree in Social Work (or similar) with healthcare experience; a graduate degree and more than five years of related work are preferred.

Qualifications

• Bachelor's degree in Social Work or similar field; 2 years of Social Worker experience, preferably in healthcare.
• Prior Social Services Director experience in a Skilled Nursing Facility.
• Graduate degree in Social Work or similar field and 5 years of related experience preferable.

Responsibilities

• Prepare residents for their future and ensure social and emotional needs are met during their stay.
• Coordinate resident discharges and participate in resident care conferences.
• Apply knowledge, skills and attention to detail to keep residents comfortable and safe.

As a Social Services Director you will prepare our residents for their future. Your work will ensure our residents receive the high standard of care they have grown to expectby ensuring our residents social and emotional needs are met during their stay, coordinating resident discharges and participating in resident care conferences. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

As a minimum, a Bachelors's degree in Social Work or similar field and two (2) years of experience in a Social Worker role preferably in a healthcare setting. Prior Social Services Director experience in a Skilled Nursing Facility, Graduate degree in Social work or similar field and five (5) years of related experience is preferable but we are willing to invest in the right candidate!
