Job summary

Novato Healthcare Center is seeking a Social Services Director to lead resident psychosocial care and coordinate social work services within a California facility.

Qualifications

• Bachelor's Degree in Social Work or a human services field; one year of supervised work experience in a health care setting working directly with individuals.
• In centers under 120 beds: High School Diploma or equivalent.
• LSW requirement in TX Centers (as applicable).
• Knowledge of Social Resources in the Community; Psycho/Social Health area preferred.

Responsibilities

• Ensure residents’ psychosocial and concrete needs are identified and met in accordance with federal, state and company requirements.
• Orient residents to the center.
• Discharge planning.
• Implement and update Resident Care Plan and Social History.
• Maintain records of outside referral.
• Assist in coordinating residents’ financial affairs.
• Communicate needs and plan of care to resident, families, responsible parties and appropriate staff.
• Participate in meetings, in-services and training as required.
• Meet with consultants as required.
• Organize and Facilitate Family Council.
• Implement and maintain a Quality Measurement and Performance Improvement Program (QMPI).
• Assist with coordination of resident room moves.
