Job summary

Novato Healthcare Center is seeking a Social Services Director to lead psychosocial care and coordinate resident services. You will identify residents' needs, orient new admissions, and oversee discharge planning in alignment with federal, state, and internal requirements.

You will maintain referrals, coordinate financial affairs, and ensure clear communication with families and staff. Participation in QMPI and family council activities is expected to support quality care across the center.

Qualifications

• Bachelor's degree in Social Work or human services plus one year supervised healthcare experience.
• High School Diploma or equivalent for centers under 120 beds.
• Must present neat appearance and be in good health.
• Strong written and verbal communication skills and good interpersonal abilities.
• Knowledge of social resources in the community; Psycho/Social health background preferred.

Responsibilities

• Identify and meet residents' psychosocial and concrete needs per federal, state and company requirements.
• Orient residents to the center and assist with discharge planning.
• Maintain records of outside referrals and coordinate residents' financial affairs.
• Communicate care plans to residents, families and staff; participate in QMPI initiatives.
• Organize Family Council meetings and coordinate room moves as needed.

Principal Responsibilities:

Social Services Director CLINICAL/ADMINISTRATIVE

• Ensure the residents’ psychosocial and concrete needs are identified and met in accordance with federal, state and company requirements.
• Orient residents to the center.
• Discharge planning.
• Implement and update Resident Care Plan and Social History.
• Maintain records of outside referral.
• Assist in coordinating residents’ financial affairs.
• Communicate needs and plan of care to resident, families, responsible parties and appropriate staff.
• Participate in meetings, in-services and training as required.
• Meet with consultants as required
• Organize and Facilitate Family Council (Participate as requested by Council Members).
• Implement and maintain an ongoing Quality Measurement And Performance Improvement Program (QMPI)
• Assist with coordination of resident room moves.

CONSUMER SERVICE

• Presents professional image to consumers through dress, behavior and speech.
