Job summary

Rockport Healthcare Services is seeking a Social Service Coordinator to assess and address residents’ psychosocial needs within regulatory guidelines. The role includes orienting residents, discharge planning, and coordinating care plans, with emphasis on communication with families and staff.

The position requires a Bachelor’s in Social Work or related field, an LSW where applicable, and weekend availability. Strong organizational skills and familiarity with community resources are essential.

Qualifications

• Bachelor's degree in Social Work or human services with 1 year of supervised health care experience.
• Must work every Saturday or Sunday.

Responsibilities

• Identify and meet residents' psychosocial and concrete needs per federal, state and company requirements.
• Orient residents to the center.
• Discharge planning.
• Implement and update Resident Care Plan and Social History.
• Maintain records of outside referrals.
• Assist in coordinating residents' financial affairs.
• Communicate needs and care plans to residents, families and staff.
• Attend meetings, trainings, and in-services as required.
• Meet with consultants as needed.
• Organize and facilitate Family Council sessions.
• Maintain QMPI program to monitor quality and performance.

Principal Responsibilities:

Social Service Coordinator CLINICAL/ADMINISTRATIVE

• Ensure the residents’ psychosocial and concrete needs are identified and met in accordance with federal, state and company requirements.
• Orient residents to the center.
• Discharge planning.
• Implement and update Resident Care Plan and Social History.
• Maintain records of outside referral.
• Assist in coordinating residents’ financial affairs.
• Communicate needs and plan of care to resident, families, responsible parties and appropriate staff.
• Participate in meetings, in-services and training as required.
• Meet with consultants as required
• Organize and Facilitate Family Council (Participate as requested by Council Members).
• Implement and maintain an ongoing Quality Measurement And Performance Improvement Program (QMPI)
• Assist with coordination of resident room moves.

CONSUMER SERVICE

• Presents professional image to consumers through dress, behavior and speech.
