Job summary

Qualifications

Responsibilities

• Identify residents' psychosocial and concrete needs per federal/state/company guidelines.
• Orient residents to the center and assist in discharge planning.
• Maintain and update Resident Care Plans and Social Histories; coordinate outside referrals.
• Assist with coordinating residents' financial affairs and communicate plans of care to families and staff.
• Organize Family Council meetings and participate as requested; support QMPI program.
• Coordinate resident room moves and participate in meetings and trainings.

Principal Responsibilities:

Social Service Coordinator CLINICAL/ADMINISTRATIVE

• Ensure the residents’ psychosocial and concrete needs are identified and met in accordance with federal, state and company requirements.
• Orient residents to the center.
• Discharge planning.
• Implement and update Resident Care Plan and Social History.
• Maintain records of outside referral.
• Assist in coordinating residents’ financial affairs.
• Communicate needs and plan of care to resident, families, responsible parties and appropriate staff.
• Participate in meetings, in-services and training as required.
• Meet with consultants as required
• Organize and Facilitate Family Council (Participate as requested by Council Members).
• Implement and maintain an ongoing Quality Measurement And Performance Improvement Program (QMPI)
• Assist with coordination of resident room moves.
