Job summary

Norwalk Skilled Nursing & Wellness Centre is seeking a Social Service Coordinator to manage residents' psychosocial needs and coordinate social services within regulatory guidelines.

You will orient residents, assist with discharge planning, maintain care plans, and coordinate referrals, financial affairs, and family communication. Strong administrative and interpersonal skills are essential for this role.

Qualifications

• Bachelor's degree in Social Work or Human Services; LSW in TX Centers preferred.
• Minimum one year of supervised healthcare experience.
• For centers under 120 beds, a High School Diploma is required.
• Strong written and verbal communication; good interpersonal and organizational abilities.]
• Knowledge of social resources in the community is preferred.

Responsibilities

• Identify and meet residents' psychosocial and concrete needs per regulations.
• Orient new residents to the center and assist with discharge planning.
• Maintain and update Resident Care Plans and Social Histories.
• Manage and document outside referrals and coordinate residents' financial affairs.
• Communicate needs and plans of care to residents, families and staff.
• Participate in meetings, trainings, and QMPI initiatives.
• Organize Family Council meetings as requested and assist with room moves.

Principal Responsibilities:

Social Service Coordinator CLINICAL/ADMINISTRATIVE

• Ensure the residents’ psychosocial and concrete needs are identified and met in accordance with federal, state and company requirements.
• Orient residents to the center.
• Discharge planning.
• Implement and update Resident Care Plan and Social History.
• Maintain records of outside referral.
• Assist in coordinating residents’ financial affairs.
• Communicate needs and plan of care to resident, families, responsible parties and appropriate staff.
• Participate in meetings, in-services and training as required.
• Meet with consultants as required
• Organize and Facilitate Family Council (Participate as requested by Council Members).
• Implement and maintain an ongoing Quality Measurement And Performance Improvement Program (QMPI)
• Assist with coordination of resident room moves.

CONSUMER SERVICE

• Presents professional image to consumers through dress, behavior and speech.
• Adheres to company standards for resolving consumer concerns.
• Ensures that all consumer/resident rights are protected.
• Organize and maintain a professional work setting
