Job summary

The Blossoms is seeking a qualified Social Worker (BSW/MSW) to support residents in long-term care settings in Arkansas. You will assess social and emotional needs, develop individualized care plans, and participate in interdisciplinary teams to ensure smooth admissions, transfers, and re-entries into the community.

Responsibilities include discharge planning, coordinating DME or home care, and collaborating with the finance team to ensure proper documentation.

Qualifications

• BSW or MSW required, or Bachelors degree in related field.
• At least one year experience in Social Work field required.
• Experience in long term care and/or sub-acute setting preferred.

Duties and Responsibilities

• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
• Identifying individual social and emotional needs
• Developing and maintaining individualized care plans
• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community
