Job summary

Continuing Healthcare Solutions is seeking a qualified professional to assist in planning, developing, scheduling, organizing, implementing, evaluating, and directing our facility’s social service and admissions programs. You will ensure the medical and social needs of residents are met with individualized care.

Responsibilities

• Assist in planning, developing, scheduling, organizing, implementing and directing our facility’s social service and admissions programs.
• Ensure residents' medically related emotional and social needs are met on an individual basis.
• Provide care appropriate to the age-related needs of residents and work as part of a team.
• Follow written and oral instructions and maintain confidentiality and safety.

Experience and Skills

The primary purpose of your job position is to assist in planning,developing, scheduling, organizing, implementing, evaluating, anddirecting our facility’s social service & admissions programs inaccordance with current existing federal, state, and local standards,as well as our established policies and procedures, to assure that themedically related emotional and social needs of the resident aremet/maintained on an individual basis.

Essential Duties and Responsibilities:

• Must possess the ability to make independent decisions whencircumstances warrant such action.
• Must possess the ability to deal tactfully with personnel,residents, visitors, family members, government agencies/personneland the general public.
• Must demonstrate the knowledge and skills necessary to providecare appropriate to the age-related needs of the residents served.
• Must be a supportive team member, contribute to and be an exampleof team work and team concept.
• Must possess leadership ability and willingness to workharmoniously with other personnel.
• Must possess the ability to minimize waste of supplies, misuseofequipment, etc.
• Must possess the ability to seek out new methods and principlesand be willing to incorporate them into existing practices.
• Must have patience, tact, cheerful disposition and enthusiasm, aswell as be willing to handle residents based on whatever maturitylevel in which they are currently functioning.
• Be able to follow written and oral instructions.
• Be knowledgeable in computers, data retrieval, input and outputfunctions, etc.
• Must not pose a direct threat to the health or safety of otherindividuals in the workplace.
• Must be able to read, write, speak, and understand the English language.

*Qualifications:

Must have at least two (2) years long term care experience in nursingposition, activities, or position that exemplifies consistent residentcommunication, on the job training provided.
