Job summary

Atrium Centers in Mansfield, NY is seeking a Social Services Assistant to support residents’ social, emotional, and psychosocial needs in a 100% employee-owned organization.

You’ll assist with care planning, documentation, and coordinating services with interdisciplinary teams under the Social Services Director. A compassionate, detail‑oriented, and collaborative approach is essential.

Qualifications

• Associate’s Degree in Human Services, Social Work, Psychology, or related field preferred, but not required (experience may be considered in lieu of degree)
• Experience in skilled nursing or long-term care setting preferred
• Strong written and verbal communication skills with residents, families, and interdisciplinary teams
• Ability to assist with documentation, assessments, care planning, and regulatory requirements under the direction of a licensed Social Services Director
• Compassionate, resident-focused approach with strong interpersonal skills
• Organized and detail-oriented with ability to manage multiple priorities
• Working knowledge of resident rights, discharge planning and psychosocial support services preferred
• Positive, can-do attitude with excellent attendance
