Job summary

Atrium Centers Frederic Nursing & Rehabilitation Community is seeking a Social Services Assistant to support the Social Services Director in implementing programs and ensuring resident well-being with dignity. You will document social, emotional, and psychosocial needs, coordinate services for residents and families, and participate in care planning with interdisciplinary teams.

Qualifications

• Associate’s Degree in a Human Services, Social Work, Psychology, or related field preferred, but not required (experience may be considered in lieu of degree)
• Experience in skilled nursing or long-term care setting preferred
• Strong written and verbal communication skills with residents, families, and interdisciplinary teams
• Ability to assist with documentation, assessments, care planning, and regulatory requirements under supervision of Social Services Director
• Compassionate, resident-focused approach with interpersonal skills
• Organized and detail-oriented with ability to manage multiple priorities
• Knowledge of resident rights, discharge planning, and psychosocial support services preferred
• Positive, can-do attitude with excellent attendance
