Atrium Centers Lexington Court Care Center in Mansfield, OH is seeking a Social Services Assistant to support residents and families. You will assist the Social Services Director with implementing social service programs and documentation to ensure regulatory compliance.

What You Bring

• Associate’s Degree in a Human Services, Social Work, Psychology, or related field preferred, but not required (experience may be considered in lieu of degree)

• Experience working in a skilled nursing or long-term care setting preferred

• Strong written and verbal communication skills, with the ability to interact professionally with residents, families, and interdisciplinary teams

• Ability to assist with documentation, assessments, care planning, and regulatory requirements under the direction of a licensed Social Services Director

• Compassionate, resident-focused approach with strong interpersonal skills

• Organized and detail-oriented, with the ability to manage multiple priorities

• Working knowledge of resident rights, discharge planning, and psychosocial support services preferred

• Positive, can-do attitude with excellent attendance
