Job summary

Atrium Centers is seeking a Social Services Assistant to support the implementation of social service programs and coordinate services for residents in a skilled nursing or long-term care setting.

You will document resident needs, participate in care planning with interdisciplinary teams, and ensure timely, compliant documentation while maintaining a compassionate, resident-focused approach.

Qualifications

• Associate’s Degree preferred; experience may substitute.
• Experience in skilled nursing or long-term care preferred.
• Strong written and verbal communication with residents and families.
• Ability to document, assess, and support care planning under supervision.
• Compassionate, resident-focused approach and organizational skills.

Responsibilities

• Support the identification and documentation of residents’ social, emotional, and psychological needs

• Provide service coordination and support for residents and families, including communication and follow-up

• Maintain accurate, timely documentation to support compliance and regulatory standards

What You Bring

• Associate’s Degree in a Human Services, Social Work, Psychology, or related field preferred, but not required (experience may be considered in lieu of degree)

• Experience working in a skilled nursing or long-term care setting preferred

• Strong written and verbal communication skills, with the ability to interact professionally with residents, families, and interdisciplinary teams

• Ability to assist with documentation, assessments, care planning, and regulatory requirements under the direction of a licensed Social Services Director

• Compassionate, resident-focused approach with strong interpersonal skills

• Organized and detail-oriented, with the ability to manage multiple priorities

• Working knowledge of resident rights, discharge planning, and psychosocial support services preferred

• Positive, can-do attitude with excellent attendance
