Job summary

NewCourtland is seeking a dedicated case management professional to address clients’ psychosocial and socioeconomic needs within a gerontology-focused program. The role involves organizing educational and social programming, coordinating referrals, and maintaining an events calendar to support participant admissions.

The position requires an Associate Degree and 2–3 years in gerontology social services, with strong communication and teamwork skills.

Qualifications

  • Associate Degree required.
  • Minimum of two to three years experience in gerontology social services.
  • Strong written and verbal communication skills.
  • Ability to identify program deficiencies/strengths and facilitate necessary programmatic change.
  • Ability to work and collaborate effectively with others.
  • Proven experience in social services, case management, or related human services field.
  • Ability to engage elderly populations and collaborate with multidisciplinary teams.
  • Working knowledge of government and community support programs is a plus.
