Job summary

Catholic Housing & Community Services is seeking a full-time Social Services Care Manager to oversee caseloads at two elderly housing sites in Collingdale and Eastwick, Philadelphia. You will engage residents, assess needs, develop care plans, coordinate services, maintain records, and promote holistic, age-friendly programming within a supportive community.

A Master’s in Social Work or Gerontology and at least three years’ experience with older adults are required; this role is with an equal

Responsibilities

Asour Social Services Care Manager, you will betheheartbeat ofour residential community and anintegral part of redefining what it means toageinplace. Inthisrole, you are a strategic partner inoursocial service ecosystem, having the ability to engage and build relationships with older adults, access digital casemanagementtools, participateincollaborativeteamsandbuildproactiveandresponsive social,recreational, and health-relatedservices and programs in a relationship-centered,age affirming environment.

• Manageacaseloadofolderadulthouseholdsattwoindependentapartment communities.
• Assessallresidentsforkeystrengthsandbarrierstosuccessful aginginplace, including physical and cognitive health/functioning,ability to manage daily living tasks, mobility, access to benefits and economic stability,
• Collaborate withresidentstodevelopresident-centeredcareplansthatfocus onwhat matters toresidents and identifies corresponding interventions and resources.
• Maintain up-to-date residentfilesinthesocialservicedatabaseincluding timelycasenotes,keyinformalandformalsupportcontacts,assessments, reassessments and care plans.
• Navigate andfacilitateaccesstoservicesandadvocate withandonbehalfof residents to community-based providers to minimize, mitigate and/or eliminate service delivery barriers.
• Createandmaintainacommunity atmospherethataddressesholisticneedsof theresidents through onsite events and coordination withexternal providers. Events and gatherings requiring planning include: holiday and birthday parties,educationalandhealthrelatedpresentations,andonsitevaccineclinic and community legal clinics.
• Maintainsocialworkinterventionandgroupparticipationdata.Participatein and utilize data analysis tools to drive and improve program planning decisions.

Position Requirements:

• Master's degree from an accredited university in Social Work, Gerontology or related field required.
• Minimum three (3) years' experience working with older adults.
• Knowledge of federal and state benefits programs and their eligibility requirements.
