Job summary

Catholic Housing & Community Services seeks a full-time Social Services Care Manager for two senior affordable housing sites—Collingdale, DE County, and Eastwick, Philadelphia County. Responsibilities include assessing needs, developing care plans, maintaining resident files, advocating with providers, and organizing onsite events while using data to improve programs.

Qualifications

• Master's degree in Social Work, Gerontology, or a related field.
• Minimum three years of experience with older adults.
• Knowledge of federal and state benefits programs.

Responsibilities

• Manage caseload for older adult households at two independent apartment communities.
• Assess residents for strengths and barriers to aging in place, including health, daily living tasks, mobility, and access to benefits.
• Collaborate with residents to develop resident-centered care plans.
• Maintain up-to-date resident files in the social service database with case notes and care plans.
• Navigate and advocate for residents to community-based providers to minimize barriers.
• Create and sustain a community atmosphere with onsite events and coordination with external providers.
• Maintain data on interventions and group participation; use data tools to guide program planning.

Inthisrole, you are a strategic partner inoursocial service ecosystem, having the ability to engage and build relationships with older adults, access digital casemanagementtools, participateincollaborativeteamsandbuildproactiveandresponsive social,recreational, and health-relatedservices and programs in a relationship-centered,age affirming environment.

• Manageacaseloadofolderadulthouseholdsattwoindependentapartment communities.
• Assessallresidentsforkeystrengthsandbarrierstosuccessful aginginplace, including physical and cognitive health/functioning,ability to manage daily living tasks, mobility, access to benefits and economic stability,

• Maintainsocialworkinterventionandgroupparticipationdata.Participatein and utilize data analysis tools to drive and improve program planning decisions.

Position Requirements:

• Master's degree from an accredited university in Social Work, Gerontology or related field required.
• Minimum three (3) years' experience working with older adults.
• Knowledge of federal and state benefits programs and their eligibility requirements.
