Qualifications

• 1+ years of experience in Long Term Care/Skilled Nursing Facility.
• Strong knowledge of the overall social services functions.
• Excellent attention to detail and ability to multitask.
• Excellent customer service skills and ability to interact with residents and families in a professional and compassionate manner.

Responsibilities

• Assist with daily living activities to support residents' physical and emotional well-being.
• Provide emotional support and companionship to residents, engaging in activities that promote socialization and relationships.
• Assist with medication management, clinical records, and other administrative tasks as needed.
• Collaborate with care team to develop and implement individualized care plans.
