Job summary

Southern HealthCare Management is seeking a Social Services Assistant to support the social service function in a nursing center. You will welcome and admit new residents, complete psychosocial assessments, care and discharge plans, and help ensure a positive patient experience while assisting with treatment plans.

This is a Monday–Friday dayshift role, working closely with the Social Services Manager to coordinate care and education for patients in a long-term and short-term care setting.

Qualifications

• Minimum high school diploma or equivalent required.
• Bachelor’s degree in social work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology and
• One year supervised social work experience working directly with individuals in a health care setting.
• One or more years’ experience in health care education/ marketing / insurance or commensurate educations preferred.
• Must be familiar with medical terminology.
• Proficient in the use of personal computer.

Responsibilities

• Welcome and admit new residents and patients.
• Complete psychosocial assessments, care plans, and discharge plans.
• Ensure a comfortable and positive experience while completing treatment plans.
• Support the Social Services Manager in daily duties.

You'll enjoy a Monday – Friday, dayshift work schedule.
