Job Summary:

At Fort Lauderdale Health & Rehabilitation Center, we are seeking a dedicated and compassionate Social Worker to join our team of healthcare professionals. As a Social Service Assistant, you will play a vital role in planning, organizing, and implementing the overall operation of our Social Services Department, ensuring that our residents receive exceptional care and support.

Responsibilities:

• Participate in discharge planning, development, and implementation of social care plans and resident assessments
• Interview residents and families to obtain social history
• Work with community resources to ensure a safe plan for patients transitioning out of the facility
• Possess the ability to seek out new methods and principles and be willing to incorporate them into existing social services

Requirements:
