Anoka Rehabilitation & Living Center is seeking a Social Services Assistant to support the Social Service Director in caring for residents and promoting their physical, mental and psycho-social well-being. Based in Anoka, MN, the role involves coordinating transfers, conducting assessments, and contributing to care planning and interdisciplinary conferences.

• Ability to work as part of a team and follow assignments.
• Ability to use computers and electronic devices for timekeeping, in-servicing and documentation.

Responsibilities

• Support Social Service Director in resident care and well-being.
• Coordinate resident transfers and discharges.
• Complete assessments for Care Planning, MDS, CAAS, PPS and other documents.
• Develop Care Plans and participate in Interdisciplinary Care Conferences.
