SUN MAR HEALTH CARE is seeking a Social Services Assistant for Anaheim Healthcare Center to support social services planning, discharge planning, and resident assessments. The role involves meeting with staff, interviewing residents and families, and referring to community agencies.

The position requires a high school diploma with 6 months of SNF social services experience preferred. Full-time, various shifts available at the Anaheim location.

Qualifications

• High school diploma or equivalent.
• 6 months experience in a Social Services Assistant position in a skilled nursing facility preferred.

Responsabilités

• Assist the Social Service Director in planning, developing, organizing, implementing, evaluating and directing social service programs.
• Meet with administration, medical and nursing staff to plan social services as directed.
• Participate in discharge planning and resident assessments.
• Interview residents/families in private settings.
• Refer residents/families to appropriate social service agencies.
• Support residents with emotional issues related to illness and admission.
• Assist in finding financial assistance and community referrals.
• Collaborate with community agencies for specialized assistance.
