Job summary

Social Services Assistant at Anoka Rehabilitation & Living Center, part of Volunteers of America National Services, supports the Social Service Director in care planning and resident well-being. The role includes coordinating transfers and discharges, completing essential assessments, and contributing to interdisciplinary conferences.

• Develops and promotes the department through marketing efforts throughout the professional and social community.

Education

Compensation: $22.00-$29.00hr (Benefit Options and based on years of experience)- Pay in Lieu Option available

About the job:

The Social Services Assistant role is to support the Social Service Director in the care and treatment of residents entrusted to the facility and to attain or maintain the highest practicable physical, mental and psycho-social well-being of each resident.

• Coordinates resident transfers and discharges.
• Completes assessments of residents as necessary for Care Planning, MDS, CAAS, PPS and other required documents.
• Develops Care Plans and attends Interdisciplinary Care Conferences for residents
• Develops and promotes the department through marketing efforts throughout the professional and social community.

Required Qualifications:

• High school diploma or equivalent education required; Bachelor’s degree in Social Work or related degree in social sciences preferred.
• Must be eighteen (18) years old or older at start date of employment.
• Ability to safely perform the essential functions of the job without harm to self or others.
• Ability to understand and follow work assignments.
• Ability to work as part of a team.
• Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.

Preferred Qualifications:

• Experience in the long-term care setting.
• Licensed where applicable by the State
