Salary Range: $50,000.00 - $52,000.00 per year

Primary Responsibilities

• Conduct assessments and develop person-centered service plans
• Support individuals’ choices, preferences, and self-determination
• Identify and connect clients to appropriate community resources
• Advocate for necessary medical, social, and educational services
• Coordinate and access waiver and non-waiver funding sources
• Empower individuals to build personal responsibility and independence
• Monitor and evaluate plans of care to ensure safety and well-being

Qualifications

Graduate from an accredited four-year college with a major in social work, psychology, sociology, or a closely related field; or a graduate from an accredited four-year college with a major in any field and one year experience as a social worker in a public or private social service agency. Experience working in Adult Foster Care, Independent Living Skills (ILS), ARMHS, or supported employment is a plus.
