Job summary

Qualifications

• Caring, compassionate attitude and a desire to help adults live with independence.
• Reliable transportation to commute to multiple sites.
• Valid driver’s license with personal vehicle and auto insurance.
• Willingness to undergo a state and federal fingerprint background check.
• Current CPR certification.
• TB test current; assistance available to obtain both if needed.
• Experience supporting individuals with developmental disabilities is a plus.

Responsibilities

• Provide person-centered instruction through planned community activities.
• Support individuals in accessing community resources and planning engaging activities.
• Take individuals on community outings for appointments, errands, or social activities.
• Track and document progress with concise case notes.
• Redirect challenging behaviors and help resolve issues constructively.
• Collaborate with a team of professionals to ensure comprehensive support.
