Job summary

Community Bridges, Inc. in Phoenix seeks a Client Stabilization Specialist to support individuals and families experiencing homelessness at shelter sites. You will stabilize arrivals, educate on rules and resources, and guide them through intake and orientation while in shelter or temporary lodging.

You will perform face-to-face visits, perimeter walks, and escalate issues to leadership. Data entry into HMIS and housing prioritization assessments are also required to coordinate services.

Qualifications

• High school diploma or GED required.
• Minimum 12 months of recovery from substance use and/or mental health disorders required.
• 1-3 years of full-time health care related work experience preferred.
• Valid identification required.
• Arizona Fingerprint Clearance Card (program specific) may be required.

Responsibilities

• Complete intakes into HMIS and provide orientation at shelter or lodging sites.
• Educate on rules, responsibilities, resources, and support stabilization.
• Conduct face-to-face visits, perimeter walks, and escalate program violations.
• Communicate needs to leadership and Client Care Coordinators for coordinated services.
• Enter data into established data systems (violations, shift notes, room checks).
• Identify housing prioritization assessments for individuals.
