Job summary

Community Bridges, Inc. (CBI) seeks a Client Stabilization Specialist to coordinate stabilization for individuals and families in shelter settings. You will conduct intakes, educate on rules and resources, and maintain oversight through face-to-face visits and perimeter checks.

You will document outcomes in HMIS, escalate issues to leadership, and help identify housing prioritization needs to support rapid stabilization and smoother transitions for clients in crisis.

Qualifications

• Highschool diploma or GED required.
• Minimum of twelve months of recovery from substance use and/or mental health disorders required.
• 1-3 years of full-time health care related work experience preferred.
• Arizona Fingerprint Clearance card required.
• CBI Peer Certification within 90 days of hire is expected.
• BHT certification is preferred but not required.

Responsibilities

• Stabilize individuals or families entering shelter or temporary lodging.
• Conduct intakes into HMIS and provide orientation on shelter rules and resources.
• Perform face-to-face visits, perimeter walks, and escalate issues impacting stabilization.
• Communicate needs to leadership and coordinators to coordinate services.
• Complete data entry in established systems and conduct housing prioritization assessments.
