Community Bridges, Inc. seeks a Client Stabilization Specialist to support individuals and families experiencing homelessness at shelter sites. You will conduct intakes into HMIS, explain rules and resources, and orient clients to shelter procedures while ensuring safety and stabilization.

You will perform face-to-face visits, perimeter walks, and escalate issues to leadership and coordinators. The role requires communication with care teams, data entry into the system, and helping identify

Qualifikationen

• Highschool diploma or GED required.
• Minimum six months of recovery from substance use and/or mental health disorders required.
• 1-3 years of full-time health care related work experience preferred.

Aufgaben

• Stabilize individuals or families entering shelter or temporary lodging.
• Conduct intakes into HMIS and provide education on rules, resources and orientation.
• Perform face-to-face visits, perimeter walks and escalate issues to leadership.
• Communicate identified needs to Client Care Coordinators for coordinated services.
• Complete data entry into the established data system, including notes and violations, and identify housing priority needs.
